Instructions for filling in application form for recipients of grants and scholarships
The insurance application form for recipients of grants or scholarships should be filled in by the person who has been awarded the grant or scholarship personally or who works on a grant or scholarship awarded to a group, in which case the group leader must also fill in the Group leader’s notification form.
The information requested on the application form is essential for insurance purposes, so please fill in all sections carefully. Appendices are not required for the most part with the exception of applications by group leaders and for grants or scholarships awarded before 1 January 2009.
Applicant
Your country of residence and the country in which the grant or scholarship work is being performed must always be specified. If your country of residence is not Finland, include also your foreign address. If you work elsewhere than in Finland, include your foreign address and further information (Section 5) about the duration of your work abroad and whether your grant or scholarship was awarded specifically for work abroad. Before you start working abroad you should clarify whether you are covered by Finnish social security. Further information is available from the Finnish Centre for Pensions: www.etk.fi.
Notify on your application whether you receive or have applied for a pension. If you receive a work disability or part-time pension, you have the chance to obtain insurance alongside your existing pension. In this case, provide a detailed estimate of your remaining work ability under further information (Section 5). The estimated amount of your remaining work ability (%) will be used as one of the criteria for determining the income for your insurance alongside your pension.
Grant or scholarship
Specify clearly the name of the issuer of the grant or scholarship and the date issued (the date of the grant or scholarship decision). Provide also exact starting and ending dates for the work period. You would be considered to be employed by the issuer if you receive wages or a salary from the issuer.
Select the field of the arts or sciences in which the grant or scholarship has been awarded. This information is for statistical purposes only. Specify the intended use of the grant or scholarship as stipulated in the grant or scholarship decision. Basic studies refer in this context to a vocational qualification, polytechnic degree, or lower or higher university-related studies or thesis work.
A group refers to a research or artist group that has been awarded the grant or scholarship and that comprises individual persons. Group members should include the name and contact information of the group leader in this section. Group leaders must also fill in a separate notification form.
Amount of grant
The gross amount of a personal grant or scholarship refers to the total amount of the grant or scholarship awarded or scholarship for the entire work period. If a portion of the grant or scholarship is intended to cover work-related expenses, such as materials, administration and travel, these should be itemised on the application form. Pension fees and social security expenses do not have to be listed here.
The total amount of a grant or scholarship awarded to a group, including itemised expenses, should be stated on the Group leader’s notification form, which should be filled in by the group leader (the individual to whom the grant or scholarship decision is addressed).
The work portion of a personal grant or scholarship is calculated by deducting expenses from the total amount. Mela uses this figure to calculate annual income according to the following formula: (gross amount of grant or scholarship – expenses) x 360 / number of workdays
Voluntary

Further information
Here you can include details about work abroad. Group leaders should submit a separate notification form as an appendix to this application.
Signature
The application must be signed and dated by you personally or by an authorised person acting by power of attorney, in which case the original power of attorney should be included as an appendix to this application. The name and contact information of the person acting by power of attorney must also be included on the application form.
